dedicated to locating and identifying hard-to-reach older persons and assisting them in gaining access to needed services:
Activities may include,
but are not limited to:
· Interactions initiated by a provider to identify persons in need
· Provision of information about services
· Provision of information about benefits

Encouraging the use of appropriate
services
