These are services that provide individuals
with information, referral and follow-up concerning community resources. Information
and Referral is a process as well as a distinct service. The center staff must
assess the need for a referral, establish contact with an agency to which a client is being referred and advise clients how
to access services, etc.
Referrals may include, but are not limited to Adult Protective Services,
Medicaid, Home Health Care, Nursing Homes, Housing, Legal Services.
